Diabetes Care

Living with diabetes can feel overwhelming. There is so much to learn, including how nutrition and physical activity affect your blood sugar, how to take your medications, and how to use a glucose monitor.

We’re here to support you. In fact, your pharmacist is a key part of your diabetes care team. Many of our 360Health Pharmacy & Wellness pharmacists have extra training in diabetes care services, including our Certified Diabetes Educators (CDE)®.

Health Coaching for Diabetes

Just like an athlete works with a coach to develop skills to excel at their sport, health coaching helps you excel at living your life - even with a chronic medical condition.  Health Coaching for Diabetes turns education into action and lets you proactively manage your diabetes and helps you avoid diabetes-related health problems, such as vision loss, heart disease, and foot complications.

Health Coaching for Diabetes gives you ongoing support to reach your health goals with personalized advice, care and skills development with our pharmacists.

We recommend you start with the Initial Needs Assessment to create a personalized care plan.  Then choose from the other services as needed to support your journey for living well with diabetes.

Initial Needs Assessment- Newly diagnosed, struggling to manage your diabetes, or time for a check up? Get a personalized care plan to help you meet your goals for medications, testing and more, with a one-on-one consultation.

Your pharmacist will:

  • Create your individualized diabetes management plan

  • Ensure your medications are working for you

  • Help you understand insulin and other injectable medications and injection techniques

  • Review glucose monitoring and other technology specific to diabetes management

  • Teach you how your blood pressure affects your diabetes, and so much more.

Medication Therapy Management - More treatment options are available than ever before. With a medication review we will make sure your medications are safe and effective, explain how they work, how to take them, and what to do if you experience side effects. If your medication is administered by injection, we will ensure you have the information you need to feel comfortable administering it.
With a full record of your prescriptions, our pharmacist can make sure your diabetes medication doesn’t interfere with other medication you may be taking, including over the counter products and supplements.  We will also work with your other healthcare providers, to make sure your prescriptions are appropriate.

Injection Technique - Get customized training from a pharmacist on how to administer, monitor and adjust insulin dosing to meet your needs.

Healthy Eating - Food is key to managing diabetes and it's natural to have questions about what to eat and when. A pharmacist offers evidence-based nutrition advice that fits your life and goals.  You’ll learn about macronutrients, portion sizes, meal planning, glycemic index, carbohydrate counting, supplements and more. 

Physical Activity - Learn how to use physical activity as a core component of your diabetes care to improve glucose control, manage weight, and achieve many other physical and mental health benefits. A pharmacist works with you to set realistic activity goals based on your personal preferences. As always, it is important to see your physician before starting any new fitness program.

Management of Hypo and Hyperglycemia - Are you concerned about your blood glucose control?  A pharmacist will advise you how to effectively prevent and manage glucose excursions by identifying triggers for highs and lows including medications, diet, exercise, stress and illness.

Self Monitoring of Blood Glucose - Learn how to test your blood glucose with a purpose.  Knowing how to track your readings will help you manage your diabetes and help to prevent complications.  There are many ways to monitor your glucose levels, from finger poke testing to using a sensor that continuously monitors your blood sugar levels.

We will assess your testing needs and help you find the best monitoring system based on your health status, insurance coverage and personal preferences. 

We’ll show you how to use the device, interpret results (like A1C, Time in Range, Random and Fasting Glucose), adjust your treatment plan for better outcomes as well as be there to troubleshoot any problems you may have along the way.

Diabetes Supplies/Technology Use - Looking for the latest technology or need a "check up" to ensure you’re using your devices properly?  Whether it’s insulin pumps and supplies, glucose strips and sensors, lancing devices, needles, syringes, or other health monitoring technology, stay informed to keep your diabetes well managed.

Sick Day Management - Being sick is no fun for anyone, but with diabetes there are some things you need to know to keep your condition in check.  A pharmacist will review how a sick day can impact your glucose control and give you guidance on how to manage a sick day plan (including a “sick day-ready kit").

Preventing and Managing Complications - Diabetes is a complex, chronic condition affecting many organs and systems in your body including the heart, kidneys, blood vessels and nerves.  Learn key management strategies, including medications and regular health checkups, and get a personalized plan to reduce your risk of complications. 

Follow Up Appointment - How are you doing with your diabetes care plan?  Schedule a follow up to check on your progress and determine next steps in your diabetes journey.  We’ll review the goals identified during your initial assessment and subsequent health coaching sessions to reinforce your health goals and fine tune your action plan going forward.
