What is Health Coaching
Just like an athlete works with a coach to develop skills to excel at their sport, health coaching helps you excel at living your life - even with a chronic medical condition. Health Coaching for Diabetes turns education into action, lets you proactively manage your diabetes and helps you avoid diabetes-related health problems, such as vision loss, heart disease, and foot complications.
Health coaching is a collaborative approach that helps individuals make sustainable lifestyle changes. Pharmacists will work directly with patients to:
- Understand their diagnosis
- Set realistic health goals
- Improve medication adherence
- Make informed choices about nutrition, activity, and self-care
